CHC43121 Certificate IV in Disability Support is currently offered through Online live stream study. This nationally

Completion of:CHC33021 Certificate III in Individual Support

OR

Completion of: CHC33015 Certificate III in Individual Support

Completion of: CHC30408 Certificate III in Disability PLUS the CHCSS00125 Entry to Certificate IV in Disability Support Skill Set.

Successful applicants will need to:

• Be able to handle physical aspects of the role.
• Be emotionally equipped to handle the role.

Work Placement:

A work placement is a requirement of this course, regardless of whether the individual is already working within the industry,the purpose of which is to facilitate and ensure workplace-based assessment. All learners undertaking this qualification will be provided with a structured learning plan that articulates the purpose of the placement, the requirements of the workplace training and assessment as they align to specific units of competency within the qualification. Learners undertaking this qualification will be required to complete and evidence of a minimum of 120 hours of work placement, the evidence of this will be captured in a Work Placement Logbook. The Logbook will be signed off by the learner, their workplace supervisor/mentor, and their Trainer and Assessor from Rise Education and Training. The Logbook is designed to capture: Shift/s undertaken as part of 120 hours work placement. Duties and task undertaken on each shift and what unit of competency they apply to The Logbook entries must be completed each day that a shift is attended, and all details must be completed. Rise Education and Training Administration maintains a list of suitable placement venues to support student to secure a vocational placement. Rise Education and Training will support and make all attempts to secure a place for the student.

Required Work Placement hours:

Students are required to attend at least 7.5 hours per/day shift work placement during a workplace session.
Students will be advised in writing or electronically and provided with a Work Placement Logbook to record their work

placement hours or work experience. Trainer/Assessor will schedule regular workplace visits, to observe and assess the

job skills and performance.
